The Dementia and Alzheimer's Log is used to identify, locate and protect those with Dementia or Alzheimer's.

How to Enroll
To enroll your loved one in the log, follow these simple steps.
  1. Fill out an application with your loved ones information. Including caretaker, family member, and doctor contacts.
  2. Attach a recent photo (shoulders up preferable)
  3. Call in with any major changes or updates.

The application and photo will be kept on file at the police department, and used in case of an emergency.
